Antimicrobials and renal failure in neutropenic patients BJID
França,Monique Sedlmaier; Lima,Stella Sala Soares; Duclou,Caroline Naback; Goulart,Eugênio Marcos Andrade; Romanelli,Roberta Maia de Castro.
Antimicrobial treatment is often indicated to neutropenic patients. Although renal failure is a common complication of many antibiotics, no information could be found in the literature defining which are the best screening criteria for detecting renal injury. In this paper, the authors aim to assess the progress to renal failure in neutropenic patients on antimicrobial use and to compare different diagnostic criteria of renal failure in association to antimicrobial agents used. This is a cohort study conducted from February to August 2006 at the Hospital das Clínicas of the Universidade Federal de Minas Gerais, which included patients with neutropenia and antimicrobial therapy for the treatment of Healthcare Associated Infections notified by the Hospital...

Fluconazole prophylaxis in preterm infants: a systematic review BJID
Rios,Juliana Ferreira da Silva; Camargos,Paulo Augusto Moreira; Corrêa,Luísa Petri; Romanelli,Roberta Maia de Castro.
ABSTRACT Objective: This article aims to review the use of antifungal prophylaxis with intravenous fluconazole in premature newborns and the occurrence of Invasive Candidiasis. Methods: This is a systematic review with search at databases: PubMed, Capes Portal, Virtual Health Library (BVS - Biblioteca Virtual em Saúde)/Lilacs, Scopus and Cochrane. The keywords used were: "Antifungal", "Candida" "Fluconazole prophylaxis" and "Preterm infants". Results: Invasive Candidiasis was evaluated in all the twelve items. In eleven of them, there was a statistically significant difference between the groups receiving prophylactic fluconazole, with lower frequency of Invasive Candidiasis, compared to placebo or no prophylaxis group. Colonization by Candida species...

Inadequate use of antibiotics and increase in neonatal sepsis caused by resistant bacteria related to health care assistance: a systematic review BJID
Silva,Ana Carolina Bueno; Anchieta,Leni Marcia; Lopes,Marianna Fischer de Paula; Romanelli,Roberta Maia de Castro.
ABSTRACT Background Technologies and life support management have enhanced the survival of preterm infants. The immune system of newborns is immature, which contributes to the occurrence of healthcare-associated infections. The overlap of several conditions with neonatal sepsis and the difficulty of diagnosis and laboratory confirmation during this period result in a tendency to over-treat neonatal sepsis. The use of antimicrobial agents is a risk factor for multidrug-resistant bacterial infections. This work aimed to perform a systematic review of the relationship between inadequate use of antimicrobial agents and increase in neonatal sepsis related to healthcare assistance, due to bacterial resistance. Methods Our population, exposition, comparison,...

Outbreak of resistant Acinetobacter baumannii: measures and proposal for prevention and control BJID
Romanelli,Roberta Maia de Castro; Jesus,Lenize Adriana de; Clemente,Wanessa Trindade; Lima,Stella Sala Soares; Rezende,Edna Maria; Coutinho,Rosane Luiza; Moreira,Ricardo Luiz Fontes; Neves,Francelli Aparecida Cordeiro; Brás,Nelma de Jesus.
Acinetobacter baumannii colonization and infection, frequent in Intensive Care Unit (ICU) patients, is commonly associated with high morbimortality. Several outbreaks due to multidrug-resistant (MDR) A. baumanii have been reported but few of them in Brazil. This study aimed to identify risk factors associated with colonization and infection by MDR and carbapenem-resistant A. baumannii strains isolated from patients admitted to the adult ICU at HC/UFMG. A case-control study was performed from January 2007 to June 2008. Cases were defined as patients colonized or infected by MDR/carbapenem-resistant A. baumannii, and controls were patients without MDR/carbapenem-resistant A. baumannii isolation, in a 1:2 proportion. For statistical analysis, due to changes...

Prognostic markers of symptomatic congenital cytomegalovirus infection BJID
Romanelli,Roberta Maia de Castro; Magny,Jean François; Jacquemard,François.
The objective of this research was to identify maternal and fetal characteristics as prognostic markers of congenital cytomegalovirus (CMV) infection. This is a descriptive study of 13 cases of congenital CMV infection referred to Institute de Puericulture et Perinatologie de Paris (IPP) from January 2005 to October 2006. Amniotic fluid puncture was performed to research CMV polimerase chain reaction (PCR). Cordocentesis and cord blood samples at delivery were also analyzed to determinate fetal platelets count, GGT, ASAT, ALAT, CMV-DNA and IgM antibody. Variables of symptomatic and asymptomatic infants were then compared. Data were analyzed by SPSS - 15.0. Mean gestational age of amniocentesis was 24.6 weeks and there was no difference of mean viral load...

Risk factors for laboratory-confirmed bloodstream infection in neonates undergoing surgical procedures BJID
Romanelli,Roberta Maia de Castro; Anchieta,Lêni Márcia; Carvalho,Elaine Alvarenga de Almeida; Silva,Lorena Ferreira da Glória e; Nunes,Rafael Viana Pessoa; Mourão,Paulo Henrique; Clemente,Wanessa Trindade; Bouzada,Maria Cândida Ferrarez.
BACKGROUND: Healthcare Associated Infections constitute an important problem in Neonatal Units and invasive devices are frequently involved. However, studies on risk factors of newborns who undergo surgical procedures are scarce. OBJECTIVE: To identify risk factors for laboratory-confirmed bloodstream infection in neonates undergoing surgical procedures. METHODS: This case-control study was conducted from January 2008 to May 2011, in a referral center. Cases were of 21 newborns who underwent surgery and presented the first episode of laboratory-confirmed bloodstream infection. Control was 42 newborns who underwent surgical procedures without notification of laboratory-confirmed bloodstream infection in the study period. Information was obtained from the...

Serum levels of vancomycin: is there a prediction using doses in mg/kg/day or m2/day for neonates? BJID
Romanelli,Roberta Maia de Castro; Anchieta,Lêni Márcia; Fernandes,Juliana Chaves Abreu; Lima,Mariana Antunes Faria; Souza,Taís Marina de; Rosado,Viviane; Clemente,Wanessa Trindade; Camargos,Paulo Augusto Moreira.
Abstract Coagulase-negative Staphylococcus has been identified as the main nosocomial agent of neonatal late-onset sepsis. However, based on the pharmacokinetics and erratic distribution of vancomycin, recommended empirical dose is not ideal, due to the inappropriate serum levels that have been measured in neonates. The aim of this study was to evaluate serum levels of vancomycin used in newborns and compare the prediction of adequate serum levels based on doses calculated according to mg/kg/day and m2/day. This is an observational reprospective cohort at a referral neonatal unit, from 2011 to 2013. Newborns treated with vancomycin for the first episode of late-onset sepsis were included. Total dose in mg/kg/day, dose/m2/day, age, weight, body surface and...
